But the details underneath that headline contain strategic signals that health plans should factor into their technology investment decisions.<\/p>\n\n\n\n

The most significant signal: CMS struck the planned risk model recalibration. The 2024 V28 model will continue unchanged into CY 2027. CMS cited skin substitute data anomalies and V28 “settling” concerns as reasons for the freeze. This is temporary. CMS explicitly stated that recalibration will return. But the freeze gives plans a stable modeling environment for at least one more year, which means the plans that optimize their coding for the current V28 coefficients now will capture the value before recalibration reshuffles the deck.<\/p>\n\n\n\n

The second signal: CMS finalized the exclusion of unlinked chart review reconciliation diagnoses for CY 2027, with a narrow exception for MA-to-MA switchers. The unlinked CRR exclusion puts immediate pressure on health plans to ensure every retrospective chart review diagnosis is linked to a legitimate encounter. Plans using fragmented systems where coding decisions happen in one environment and encounter data lives in another can’t verify linkage efficiently. The verification requires cross-referencing chart review output against encounter submission records in real time, something that’s manual and error-prone across disconnected systems.<\/p>\n\n\n\n

When a coder identifies a diagnosis through retrospective review, the system checks whether that diagnosis links to a submitted encounter.
